Release of Liability Form

Release of Liability:

I, the undersigned, being the legal guardian of the participant(s) listed above, hereby agree to release, indemnify, and hold harmless F3 Academy, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by the participant(s) or property belonging to the participant(s), whether caused by the negligence of the releasees or otherwise, while participating in F3 Academy.

I understand that participation in the program involves inherent risks, including but not limited to, the risk of injury from activities involved in the program. I acknowledge that I have voluntarily chosen to allow the participant(s) to participate in the program with full knowledge of the risks involved.

Media Release:

I hereby grant F3 Academy the right to take photographs, videos, and other media of the participant(s) listed above during their participation in the program. I authorize F3 Academy to use and publish these images and videos for advertising, promotional, and educational purposes, including but not limited to, website content, social media, brochures, flyers, and other marketing materials.

Inclement Weather Policy:

 

In the event of inclement weather, including but not limited to, thunderstorms, heavy rain, extreme heat, field closure, air quality, or any other conditions deemed unsafe by F3 Academy staff, sessions may be canceled or postponed. F3 Academy will notify participants and guardians of cancellations or changes to the schedule via email, or social media as soon as possible.

Refund Policy:

 

Please note that refunds for cancelled sessions will not be provided. However, participants will be offered the opportunity to attend subsequent sessions or receive credit towards future training blocks.
